How to fill out a form in office

Sometimes clients forget to fill out forms. When that happens, ClinicSense makes it easy to fill out forms in office. This guide explains how.

Note: Generally, you won't need to manually send forms. Intake forms are usually sent automatically. However, it's sometimes useful to have a manual option.

This video demonstrates how to fill out forms with a client in-office.


Generating new forms

To fill out a form in the office:

  1. Go to the Clients page
  2. Search for and select the client in question
  3. Click their Forms tab
  4. Click the New Form button

  5. Select the forms to fill out
  6. If you'd like the client to fill out forms...
    • ...on their own device, click Email Forms and ask them to check for a link in their inbox
    • ...on your own device, click Fill Out Now and follow the steps in the Fill Out Now section, below

Fill Out Now

To fill out forms on your own device:

  1. Follow steps 1 though 6 in the Generating new forms section, above
  2. Select a Who is filling out the form? option

  3. Either complete the form, or hand your device to the client to fill out the form
