How to create new intake form templates

Easily create custom intake form templates to collect the right client info for your practice.

Create a new form

  1. Go to Setup, then Form Builder
  2. Click Add New at the bottom left

  3. Customize the first question by choosing the question type from the drop-down menu

  4. Enter your question text and, if needed, answer options
  5. Toggle the "Add display logic" option, or check the "Required" and "Office use only" boxes
    • Display logic determines whether questions are shown or hidden based on previous answers
    • Required and Office use only let you control how the form is filled out

  6. Click + Add Question, at the bottom center

  7. Alternatively, click on the + button below an existing question to add directly below it

  8. To rearrange questions, click the up/down arrows. To delete, click the trash icon

  9. Click Save

Adjust option settings

  1. Click Options in the top right corner
  2. Choose how often the form should be updated
  3. Select which services require this form to be filled out
  4. Choose whether to attach the completed form to treatment notes

  5. When finished, click Save Template in the bottom right corner
