Guide
- Go to Setup, then Services
- Use the Select Service drop-down, then go to the Forms tab
- Under Which treatment note template(...)?, select your preferred option
- When done, click Save at bottom right

- The Forms tab also lets you pick which intake forms should be sent for this service
