Creating a new SOAP note template enables you to create a SOAP note that suits your service's needs.
Navigate to the Treatment Note Builder
- Click on Setup
- Select Treatment Note Builder
Adding a New SOAP Note Template
To add a new form, click the Add New button at the bottom left.
Adding Questions to SOAP Note Templates
You are able to add additional questions to your SOAP note templates by scrolling to the bottom and selecting the Add Question button.
Once you click on the button, you can:
- Choose your question type (i.e. short answer, multiple choice, etc)
- Type in your question
- Select if you'd like to enable display logic (more on that below)

Save your question
- Finally, Save the template
Display Logic
Enabling display logic will allow you to add a condition to display the question only if the condition is true.
In the example below, the question "Where does the patient feel pain?" will only display if the practitioner answers "Yes" to the previous question, which was "Is the patient experiencing pain?"
This means that practitioners will not be asked questions that are not relevant. If we know they are not experiencing pain, then there is no reason to ask where the patient feels pain.
Changing the Order of Questions On The Form
To move a question up or down the form, you can click on the up or down arrows at the top right of the question you want to move:
Editing a Question
To edit a question, click on the pencil icon at the top right of the question you want to edit:
Deleting A Question
To delete a question, click on the trash bin icon at the top right of the question you want to delete:
Previewing Your SOAP Note Template
You can preview your SOAP note template by clicking on the Preview button at the bottom left of your template within the Treatment Note Builder.
